This guidance is based on the Ontario Science Table Recommendations and is supported by the Infectious Disease Service and Pharmacy Department at William Osler Health System.
If prescribers wish to prescribe outside of these guidelines as outlined on the forms, this requires consultation with the Infectious Disease (ID) service. It is unlikely to be supported but this is the appeals mechanism. Pharmacists do not have authority to dispense these medications outside of the criteria on the form unless ID has approved the utilization of the agent in that setting.
